
Jill Beardsley, MA, CAGS
Jill is the co-owner of Case Management Professionals, Inc. She has over 30 years of experience in case management and administration. This includes founding Community Support Associates, Inc. in 1993, an agency which provides residential, case management, and outreach services to adults with disabilities, of which she is still a principal. She has coordinated facility based and community services for individuals with stroke, aphasia, dementia, and traumatic brain injury. Jill is a licensed rehabilitation counselor in the state of Massachusetts and is a member of the Aging Life Care Association.
Katrina Fallon, LCSWKatrina has been working for Case Management Professionals since the company became a division of Community Support Associates in 2018. Katrina has over 15 years of experience in the social work and education fields, working with varying populations and needs. From preschoolers to families, adults and elders, Katrina has worked to support individuals and teams in multiple settings such as schools and after school programs, residential programs, state agencies, addiction rehabilitation centers, skilled nursing facilities, assisted living communities, and in outpatient counseling. Importantly, Katrina has coordinated facility-based and community-based services for individuals with dementia, stroke, aphasia, traumatic brain injuries, multiple sclerosis, and other physical and intellectual variances.
Katrina received her bachelor's degree in Art with a professional concentration in art therapy from The University of Tampa. She followed with a master’s degree in social work from Bridgewater State University. She is licensed in Clinical Social Work and is a member of the Aging Life Care Association. |
Dawn M. Black, RN Dawn joined Case Management Professionals in October 2018 and brings both an extensive clinical background and a genuine interest in families and developing relationships with her clients, their families and other professionals to achieve their long-term goals. Dawn has worked both privately and professionally in nursing and geriatrics on Cape Cod for the past 20 years. Dawn has always been involved in advocacy and providing the best care for her clients and their families. She was certified as a Certified Senior Advisor in 2002, and has worked in home care marketing, sales and care management, as well as assisting patients and families with end of life services through both hospice and palliative care. Dawn has worked in many clinical settings in addition to her extensive work in home care. She is currently pursuing an advanced degree in Nursing through Cape Cod Community College and University of Massachusetts Boston.

Mary G. Ciminelli, M.S.Mary has been working for Case Management Professionals since January 2019. Prior to joining Case Management Professionals, Mary worked as an Independent Geriatric Care Manager. She also has over 30 years of experience working in the fields of vocational rehabilitation and as a Counselor and Administrator in Higher Education. Throughout her professional career she has worked with a diverse population of individuals ranging in age from eighteen to 91+ years. Mary's specialization has been working with individuals with disabilities and assisting them in achieving their highest level of independence. As a Care Manager, Mary understands the importance of building relationships with not only her clients, but with their families, significant others and the various professionals who work to provide optimum care to enhance the client’s life. Mary received her bachelor's degree in Social Welfare from The State University of New York at Buffalo and her Master's of Science in Rehabilitation Services Administration with a specialization in Work and Personal/Social Adjustment from the University of North Texas. She was a Licensed Professional Counselor in the State of Texas from 1984-2017 and is a member of the Aging Life Care Association.
